What the enquiry form collects
The form collects a parent or carer’s name, phone number, email address, optional child age, optional town or postcode, contact preference and message. This is used to respond to your enquiry and take steps towards arranging a service.
Please do not send detailed health information through the enquiry form. Any information needed for care will be discussed through an appropriate channel.
Lawful basis and retention
Enquiry information is used where necessary to respond before entering into a service agreement and for legitimate business administration. It is kept only as long as needed for the enquiry, legal obligations and appropriate record keeping.
Your rights
Under UK GDPR, you may have rights to access, correct, erase, restrict or object to use of your information, and to complain to the Information Commissioner’s Office.
